What Is Form-V?
Form-V is the official Marriage Certificate prescribed under the Delhi (Compulsory Registration of Marriage) Order, 2014, read with the Hindu Marriage Act 1955 / Special Marriage Act 1954. It carries:
- Government of Delhi emblem
- SDM signature and seal
- Register volume and page number
- Both partners' details
- Witnesses' signatures
- Date and place of marriage

Apostille — When & Why
India is a member of the Hague Apostille Convention 1961. For use in 125+ Hague member countries, the Form-V needs only MEA Apostille (₹3,500 service from our office) — no further embassy step.
For non-Hague countries (UAE, China, Saudi Arabia, Kuwait, Qatar, Bahrain, Egypt), embassy attestation is needed — we do this for ₹4,500.
Form-V is NOT Required For:

Form-V's authenticity can be verified by:
- Visiting SDM Tis Hazari and checking the marriage register against the volume/page number on the certificate
- Online RTI to the Delhi Revenue Department
- For NRI use, the receiving country's embassy may verify with MEA Delhi
